Digital Imagery Consent Form

As part of your iPET Network qualification, you are required to obtain and use images and/or video for the purpose of use as evidence against your assessment(s). These images may contain your own image and/or that of your peers. Wherever possible, inclusion of images containing others should be minimised through avoidance or blurring/obscuring the face of others.

In line with the General Data Protection Regulation (GDPR), 2018, the number of images will be kept to the minimum required in order to fully authenticate the assessment and will be stored only within the secure, password protected e-portfolio. Where a paper-based, or other electronic, portfolio is used, this must be kept in a secure manner on a password-protected device.

Only yourself, your Tutor and selected iPET Network staff members will have access to these images and they will not be used for any purpose other than verifying your identity and the authenticity of the assessment. Images will be stored for a minimum of 3 years. The storage of evidence complies with your Training Provider Terms and Conditions of service including the Privacy Policy.

Images captured for the purposes of your qualification are owned by your Training Provider. By signing this form you agree not to publish these images on social media or using any other means.

To provide this consent, you must be over 18 years old. Those Candidates under 18 years of age must have this form signed by a Parent or Legal Guardian.
